Missing elements in democratized form

Hi- I'm experiencing an issue where I can't fill out the Privacy and License Type sections of the democratiz3d form because they don't appear to exist, and I don't think I can proceed to the step where I convert my nrrd file to an STL. Has anyone else had this problem? Attached are a screenshot of the form from the getting started section of this site and a screenshot of what I'm seeing

I just saw a text somewhere saying that the app is down since 2022 but I can't find it anymore. So I'm affraid this is not going to work 😕
